After youâve had breast reconstruction surgery following a mastectomy or lumpectomy, thereâs one more possible step. Youâll need to decide if and how youâd like to have your nipple — and areola, the colored area around it — reconstructed. A Baltimore-area tattoo artist is making a name for himself inking 3D nipples onto the chests of breast cancer survivors.

For many women, Vinnie Myersâ tattoo shop in a nondescript strip mall in Finksburg, Md. is the very last stop of a âphysically and emotionally taxingâ journey.

âIâm the last person they see in that big battle,â Myers told the Daily News. âThey need nothing else. They beat it, theyâre done.â

As one of his customers put it, the nipple and areola tattoos are âthe icing on the cake.â

Lillie Shockney, a nurse at Baltimoreâs John Hopkins Breast Center, went to Myers after she saw his work on one of her patients.

When he was done, âhe turned my chair so I could see what I looked like in the mirror, and I burst into tears because they truly look real,â she told the Daily News.

Typically in the breast reconstruction process, a nurse or physicianâs assistant does the dermal abrasion that creates the appearance of nipples and areolas.

When Shockney had it done about ten years ago, she had three colors to choose from: light tan, dark brown or salmon.

âI donât think anyoneâs breasts should be referred to as fish, but that was the most common color,â she said.

Shockney chose salmon, and the result âlooked like two pancakes.â

While color options have since improved, itâs hard to match Myersâ decades of experience in color-matching and shading.

He does about 4 to 5 nipple and areola tattoos a day, 5 days a week.

What Should I Do During The Healing Period

For the first 10-14 days after the tattoo, it is important to keep the site clean and apply a healing ointment twice per day. Avoid scratching or picking at the tattoo while it is healing.

What Happens During Each Areola Treatment

STEP 1: THE COLORColors are custom blended to match your skin tones. They are non-toxic and made from natural mineral-based pigments.

It takes about 4 weeks for the color to completely settle and stabilize in the skin. Every 4 weeks youll return for more pigmentation until you have had a minimum of 4 visits on each patch of Areola. If projection is not important, then 3D nipples tattoos may be right for you! There are two main types, and both are often referred to as 3D tattoos, so its important to know the differences.

MicropigmentationMicropigmentation is a process where a micropigmentation specialist or nurse injects pigment, not ink, into the dermal layer of the skin. The procedure is typically performed at a clinic. The procedure takes about an hour and there are minimal risks that include scarring, irritation, uneven color, or infection.

Micropigmentation does fade, and touch-ups are recommended about every two years. But the advantage of that is that if things dont turn out 100% perfect, they can be adjusted. As your skin changes over time, your coloring can be adjusted so that your nipples still appear in the correct place. It usually takes a couple of sessions to get the coloring just right, and multiple pigments are blended to create an impressively realistic 3D look.

The Nipple Tattooing Treatment

On procedure day, patients should arrive with clean skin. No moisturizer or numbing agents should be applied before nipple tattooing.

A loose shirt that opens in the front should be worn to the appointment. A hospital gown may be provided as an alternative.

Patients should have a bra or camisole to wear after treatment to help hold dressings in place.

Before beginning the tattoo, the aesthetician will clean and disinfect the skin. The placement, size, and pigment discussed during the consultation will be reviewed and approved by the patient.

What Should You Expect Afterward

SLIGHT SWELLING and redness following the procedure and the skin may feel tight and tender. These symptoms will ease within 1 7 days depending on how sensitive your skin is. You can take Tylenol or ibuprofen if needed.

For 2 weeks DO NOT expose your tattoos to dust or dirt. DO NOT exercise or cause sweat in the area of your tattoo. Just try to keep sweating to a minimum if possible.

PEELING or scabbing may begin in about a week. Using Tegaderm or Saniderm will help minimize scabbing.DO NOT PICK, PEEL, RUB OR SCRATCH the epithelial crust, ALLOW IT TO FLAKE OFF ON ITS OWN, otherwise your color may heal unevenly and you risk infection. It is important that the healing process takes its natural course! As the pigmented area of the skin heals and dries, it forms a scab. This can last for 14 21 days and the dead cells will come off as healing takes place. This ugly phase of healing can feel like its going to last forever, but it will get better! Scarring and possible infection can occur if the scabs are removed or knocked off prematurely resulting in color loss. Allow them to fall off naturally to preserve color.

Some itching is normal. You may consider taking Benadryl if that helps or using Benadryl gel, rubbed into the skin around the areola, but not on it.

You will then notice slight fading of the pigment and softening of the color. Your tattoo may have a waxy or shiny appearance when it first peels and this is normal.

A tattoo artist in a seersucker suit, bow tie, and alligator shoes in New Orleans has been gaining the attention of women and surgeons not for his clothes but for his unique 3D tattoos. Vinnie Myers, a Maryland native, moved to New Orleans to help women renew their sense of femininity and empowerment post-mastectomy. Myers and Dr. Scott Sullivan at the Center for Restorative Breast Surgery in New Orleans have teamed up to help women recover from breast reconstructive surgery by designing 3D nipple tattoos, or nats, which give the appearance of a real nipple.

I wouldve never thought that I would be doing what Im doing now, said Myers in a The New York Times video that follows Caitlin Kiernan, science fiction author, through her journey of nipple tattoos.
